The Chinese Community Social Services Centre Inc. has been the only Chinese-specific service provider of home care packages in the State of Victoria since 1995. Our program is funded by the Australian Government to provide culturally and linguistically appropriate support service for Chinese elderly people, so as to enable them to live in their own home and spend time with their family, friends and community for as long as possible. We have had more than 20 years of experience and proven record in delivering home care packages for consumers in the Eastern, Northern, Southern and Western Metropolitan regions.

Home Care Package Levels
There are 4 levels of care packages to support consumers with:
This program is funded by the Department of Health, and each level of packages will have different levels of funding. Level of Package is determined by the Aged Care Assessment Service.
Types of Services
The types of services provided under a Home Care Package will depend on the consumer's needs, choices and preferences. Our Case Manager provides one to one consultation to you and/or your carer to develop an individual care plan to match your needs and preferences.
The Short-Term Restorative Care (STRC) Program is a government funded, new and innovative program. It is delivered in the form of goal oriented, high intensity and multidisciplinary package of services, aiming to optimise the functioning and independence of older people and to minimise functional decline.
Consumer Directed Care
All our home care packages are delivered with a “Consumer Directed Care” (CDC) model. CDC gives people a greater say and more control over the design and delivery of the care services provided to them. The following principles underpin the operation and delivery of packages on a CDC basis:
To you as a consumer, CDC means:
Fees
The fees are depending on Government’s income assessment. Consumers with above pension income will be required to pay a higher fee.
Consumers who have financial hardship can apply for hardship supplement.
