The Chinese Community Social Services Centre Inc. has been supporting Chinese elderly living at home in Eastern, Northern, Southern and Western Melbourne Metropolitan regions since 1995. Our Home Care Packages Program is funded by the Australian Government to provide culturally and linguistically appropriate support service for Chinese elderly people, so that they could continue living at own home and spending time with their family, friends and community for as long as possible.

 

Types of Services

The types of services provided under a Home Care Package will be based on your needs, choices and preferences. Our Case Manager provides one to one consultation to you and/or your carer to develop an individualised care plan to match your needs and preferences.

 

Home Care Package Levels

There are 4 levels of care packages to support you with:

    • Level 1 – basic care needs
    • Level 2 – low level care needs
    • Level 3 – medium level care needs
    • Level 4 – high level care needs

This program is funded by the Department of Health, and each level of packages will have different levels of funds. Level of Package is determined by the Aged Care Assessment Service.

 

Here are some of the services we offer:

1. Care Services:

      • Personal care (shower, personal hygiene and grooming)
      • Assistance with daily living activities
      • Nutrition, hydration, meal preparation and special dietary requirements (such as assistance with oxygen or enteral feeding)
      • Management of skin integrity (such as prepare bandages, dressings and skin emollients)
      • Continence management
      • Mobility and dexterity (such as providing aid and equipment)

2. Support Services:

      • Case management
      • 24 hours emergency support
      • Home care (such as house cleaning, laundry)
      • Gardening
      • Assistance with medication management
      • Rehabilitative support
      • Shopping and transportation to medical appointments and social activities
      • Minor home maintenance and modifications
      • Carer respite

3. Clinical Services:

      • Nursing care provided by registered nurses
      • Allied health services (by our selected Chinese speaking physiotherapist, occupational therapist, podiatrist and, dietician)

 

Consumer Directed Care (CDC)

CDC gives you a greater say and more control over the design and delivery of the care services provided to you. The following principles underpin the operation and delivery of packages on a CDC basis:

    • Consumer choice and control
    • Rights
    • Respectful and balanced partnerships
    • Participation
    • Wellness and re-enablement
    • Transparency

To you as a consumer, CDC means:

    • your right to individualised services is respected. You can take control of the service and set goals to achieve through the package.
    • your wellness and re-ablement is our important goals and we will work with you to achieve your dreams and aspirations.
    • you will be informed about the funding available to you and how it is spent by giving you individualised budget and monthly statement.

How do I Access A Home Care Package?

    • You, a family member, a friend or a support worker can contact My Aged Care (MAC) Centre and make a referral. Call MAC on 1800 200 422 or visit their website on www.myagedcare.gov.au
    • If you do not speak English, you can contact Translating and Interpreting Service (TIS) on 131 450. You can use TIS which is free of charge to call My Aged Care. When you call TIS, an operator will ask you what language you need. Tell the operator in English the language you speak. The operator will then connect you with an interpreter who speaks your language and help you to call My Aged Care Centre.

What fees will I be charged?

You may be asked to pay an income tested care fee based on Government’s income assessment. If you are receiving maximum aged pension, you will not be asked to pay an income tested care fee.

If you are experiencing financial hardship, you can apply to Services Australia for financial hardship supplement.
